DALLAS, TX -- (Marketwire) -- 01/31/13 -- Phytel, the leader in automated, provider-led population health improvement, announced today that Tom Frosheiser has joined the company as Senior Vice President of Operations, reporting to CEO Steve Schelhammer. Frosheiser brings to the position more than 25 years of operations and finance experience in a variety of senior leadership roles within high-growth healthcare companies.

Frosheiser was most recently General Manager of Accordant Health Services, Inc., a CVS/Caremark Company based in Greensboro, NC. At Accordant, Frosheiser directed all operations, finance and sales activities, helping grow the specialty chronic disease management company from early stage to profitability. Frosheiser also led Caremark's CarePatterns disease management division.

Prior to becoming General Manager of Accordant, Frosheiser held COO, CFO, and other Finance positions at Accordant; HemoCleanse, Inc., a medical device company; Quantum Health Resources, a home health and specialty pharmacy distribution company; and PacifiCare Health Systems, a healthcare company acquired by United Health Group in 2005. He has specific expertise in operationalizing all facets of population health management including data integration and analytics, message delivery via multiple communication mediums and care team organization and execution.
